Remote Monitoring for COVID-19 

  

We are collaborating with Vivify Health to provide free screening and remote monitoring for colleagues
who may be at risk or symptomatic for COVID-19.  Our highest priority is the health and safety of all our 
colleagues and patients. Remote monitoring is one of the many ways we are working to more effectively
protect you and the communities we serve during this time.

Step 1: You will receive a text message to download the Vivify Health app. Tap the link in the text and it will take you directly to the app store to download the app.

 

Step 2: Once downloaded, you will need to answer a few brief questions for COVID-19 within the Vivify Health app.

 

Step 3: Next, you will be prompted to enter information related to any current symptoms.  You will also be asked to take your temperature and submit the result

Step 4: Following the completion of steps 1-3, you will receive ongoing push notifications from the Vivify Health app to input your symptoms and temperature. It is very important to check in when prompted so our clinical staff can monitor your symptoms.  Please check your settings to ensure push notifications are turned on for the Vivify Health app so you know when to input your information each day.

 

FAQs 

What happens if my symptoms worsen?
Our clinical staff are monitoring the information you submit through the Vivify Health app Mon-Fri 7am-7pm. If your input suggests further intervention during these times, we will send you a secure text message or phone call and direct you to the best care setting. If you have concerns about your worsening symptoms outside of these hours, please contact your provider directly.

 

What if I need to contact my care team
Your care team is available Mon-Fri from 7am-7pm to answer any questions you may have. Please use the message and call options available within the Vivify Health app to contact your care team during these times.

 

What if I have questions on how to use the app?

How long will I need to monitor my condition remotely?
We ask patients to monitor their health for 14 consecutive days

 

When can I return to work?
After your monitoring period ends, the care team will work directly with your facility employee health nurse to assess when you are ready to return to work.
